Reactivations
As stated in Minnesota Statutes, Section 148E.080, you may reactivate a temporary leave license, emeritus license, expired license, or voluntarily terminated license as follows:
Reactivation from a temporary leave or emeritus license.
To reactivate a license from a temporary leave or emeritus status, a licensee must do the following no more than five years following the temporary leave or emeritus effective date:

request must be submitted in writing via email, fax postal service and include licensee's current mailing address.complete an application form specified by the Board;

submit a Supervision Plan, if required;

pay the reactivation of a temporary leave or emeritus license fee specified in Section 148E.180; and
-
pay the wall certificate fee in accordance with Section 148E.095, subdivision 1, paragraph (b) or (c), if the licensee needs a duplicate license.
Reactivation of an expired or voluntarily terminated license.
To reactivate an expired license or voluntarily terminated licensee, a person must do the following within one year of the effective date of expiration or voluntary termination:

complete an application form specified by the Board;

document compliance with the continuing education requirements that were in effect at the time the license expired;

document compliance with the supervision requirements, if applicable, that were in effect at the time the license expired; and

pay the reactivation of an expired license fee specified in Section 148E.180.
